New Payment Practices/Expectations for 2022

Hello,

With 2021 ending, many of you will be starting new deductibles and maximum out of pocket amounts beginning 01-01-2022.  Some of you may also be starting with completely new insurance policies or even with a different insurance company than what you had in 2021.

If you receive a new insurance card (even if it’s from the same insurance company), please make sure to let us know.  You will need to either bring it in so that we can make a copy of it or send us a copy of the front and back of the card as soon as possible.

If you have a copay, please be prepared to pay the full copay at the time of service as outpatient services such as appointments with a therapist or nutritional counselor may have a copay.  If you have a deductible, please be prepared to pay at least $50.00 at the time of service for each appointment you have until your deductible is met.  Those payments are also due at time of service.

Monthly payment plans can be made for outpatient services. A credit card MUST be on file to implement a payment plan.

For IOP, PHP RTC service only:

Per Evolve LLC payment guidelines, 50% of your remaining deductible is due prior to services being provided for all higher level of care programs.  After that, a monthly payment plan can be made for up to 4 months.  If you or your child are in our PHP, IOP or Residential programs, you will be required to keep a credit card on file with us.

If you are staying in our Transitional Living house (for PHP only), you will also be required to keep a credit card on file with us.  In all higher levels of care and transitional living cases, the credit card will be automatically charged for the week every Friday (unless another payment plan has been approved).  An invoice will be sent either via mail or email weekly when the credit card is charged.

For Oshkosh clients:

You will be required to have a credit card on file with us as we don’t have a receptionist on site to take payments.

For Virtual sessions:

You will be required to have a credit card on file with us so that payments can be taken at time of service.

If you would like to put a credit or debit card on file with us to be automatically charged for every outpatient visit, please either speak with the administrative assistant while you’re in the office or give them a call and they can have you fill out the appropriate forms.  If you would like us to have a credit card on file only to be charged at your discretion, that is also an option.  However, if you choose that option, any copay or deductible would still require a payment at time of service for each appointment.
